CONSENT TO BECOME A PARTY PLAINTIFF

Fair Labor Standards Act, 29 U.S.C. § 216(b)

I consent to be a party plaintiff seeking unpaid wages, liquidated damages, and all other relief permitted in law or equity (“damages”) against Davita, Inc. (“Defendant”) and all entities and/or individuals that may jointly be liable with Defendant for my claimed damages.

By joining this lawsuit, I designate the Named Plaintiff(s) and attorneys engaged by the Named Plaintiff(s) (and any other persons those individuals designate as necessary) as my representatives to make all decisions on my behalf, to the extent permitted by law, concerning the method and manner of conducting the case including, but not limited to: case strategy, negotiation, settlement, and all other matters pertaining to this lawsuit. I agree to be bound by any agreement entered into between the Named Plaintiff(s) and the attorneys engaged by the Named Plaintiff(s).

This consent is intended to be filed in order to recover any unpaid wages owed to me by Defendant whether this consent is filed in this action or in any subsequent action that may be filed on my behalf for such recovery, and that this consent may be used in this case or in any subsequent case/arbitration as necessary.

I choose to be represented by the law firms of Shellist Lazarz Slobin, LLP, Forester Haynie, PLLC and any other attorneys with whom they may associate.
